Oliver Hazard Perry sınıfı fırkateyn, ABD Deniz Kuvvetleri'nde hizmet etmiş olan fırkateyn sınıfı. Adını 1812 Savaşı'nda ABD Deniz Kuvvetleri'nin zaferine katkıda bulunup "Hero of Lake Erie" ("Erie Gölü Kahramanı") olarak anılan Komodor Oliver Hazard Perry (Matthew Calbraith Perry'nin ağabeyi)'den almıştır.
FFG-12 adını II. Dünya Savaşı'nda ölen komutan George Philip'ten almıştır. FFG-12 1980 yılında denize indirilmiş, 2003 yılında hizmetten alınmıştır. FFG-14 ise adını Amiral John H. Sides'tan almıştır. 1981 yılında hizmete giren FFG-14 İran-Irak savaşı sırasında İran Hava Yolları'na ait IR-655 sefer sayılı yolcu uçağının vurularak 60'ı çocuk 290 yolcunun ölmesi olayına dolaylı olarak karıştı. Sides, bu sırada İran karasularındaydı. Bu sebeple ABD hükûmeti İran'a 61,8 milyon dolar tazminat ödedi.
