Santral toleransın nihai sonucu, otoreaktif olmayan lenfosit popülasyonu oluşturulmasıdır.[1]
B lenfosit toleransı değiştir
Kemik iliğindeki olgunlaşmamış B lenfosit, bireyin kendi proteinlerine ("non-self" protein) bağlandığında negatif seçilime uğrar.[1]
Doğru fonksiyon gösteren bir B lenfosit, "non-self" olmayan proteinleri ya da patojen ilişkili moleküler paternleri (PAMP) tanımalıdır.[2]

T lenfositler için santral tolerans timüs bezinde gerçekleşir.[2] Söz konusu mekanizma pozitif ve negatif seçilimi içerir.[1]
T cell receptors must have the ability to recognize self major histocompatibility complex (MHC) molecules with bound non-self peptide.[2]
T lenfosit toleransının basamakları [1][3]
- Pozitif seçilimde T lenfositlerin MHC-protein kompleksine belirli bir afinite ile bağlanması kontrol edilir. Eğer T lenfosit MHC sınıf 1 ya da MHC sınıf 2 ile oluşmuş komplekse bağlanamıyorsa, hayatta kalma sinyalleri almaz ve apoptoz ile öldür. Ancak T lenfosit belirli bir afinite göstererek MHC-protein kompleksine bağlanmışsa, hayatta kalma sinyalleri alır.
- T lenfosit bağlanabildiği MHC sınıfına göre CD4+ ya da CD8+ T lenfosite dönüşür.
- Depending on whether the T cell binds MHC I or II, it will become a CD8+ or CD4+ T cell, respectively.
- Positive selection occurs in the thymic cortex with the help of thymic epithelial cells that contain surface MHC I and MHC II molecules.
- During negative selection, T cells are tested for their affinity to self. If they bind a self peptide, then they are signaled to apoptose (process of clonal deletion).
- The thymic epithelial cells display self antigen to the T cells to test their affinity for self.
- Transcriptional regulators AIRE and Fezf2 play important roles in the expression of self tissue antigens on the thymic epithelial cells in the thymus.
- Negative selection occurs in the cortico-medullary junction and in the thymic medulla.
- The T cells that do not bind self, but do recognize antigen/MHC complexes, and are either CD4+ or CD8+, migrate to secondary lymphoid organs as mature naïve T cells.
Regulatory T cells are another type of T cell that mature in the thymus. Selection of T reg cells occurs in the thymic medulla and is accompanied by the transcription of Foxp3. T reg cells are important for regulating autoimmunity by suppressing the immune system when it should not be active.[4]
